*Please note, in the "Special Accommodations" field, if you require a vegetarian, non-red meat, or Kosher alternative entrée, or have any other dietary restrictions.

All attendees will be required to show proof of COVID-19 vaccination or a negative test upon arrival. To ensure a smooth and expediated entrance into the Gala, please follow the *steps below to sign-up with Bindle and set-up your event pass for JDRF Illinois One Dream Gala. We will also accept paper credentials, such as a CDC vaccine card or a print-out of a negative COVID-19 test from a CLIA-approved lab.

Step One: Download the free Bindle app on the Apple App Store or Google Play Store. Search “My Bindle” and download onto your smartphone.
Step Two: Create an account and upload your vaccine information and/or negative test results.
