Check the box for "I agree to pay future charges, when I authorize them, according to card issuer agreement."

As a convenience, you may pre-register a credit card on the submission page to charge your purchases at event Illinois One Dream Gala 2021.

I authorize Breakthrough T1D - Illinois - 4160 to use the credit card entered on the submission page at event Illinois One Dream Gala 2021. Once the transaction has completed successfully, a confirmation receipt will be sent to the email address entered.
